- Completed fellowship in sports medicine, adult reconstruction, pediatric orthopaedics or orthopaedic trauma
- International applicants
- Institution policyYes - non-US citizens may apply, with ECFMG J-1 sponsorship and a NYS licence or Limited Permit

- Institution policyfellows must maintain appropriate immigration status; non-US citizens apply to ECFMG for J-1 visa sponsorship; immigration paperwork must start at least six months in advance

- Institution policycurrent New York State licence and registration required; non-US citizens only may instead work with a NYS Limited Permit (start at least six months ahead); Foot & Ankle, Hand, Shoulder, Sports Medicine and Regional Anesthesia fellows need a full NYS licence to rotate to the HSS Ambulatory Surgery Center
